Remote Patient Monitoring

A Healthcare Tool for Your Peace of Mind

TCCH is offering Remote Patient Monitoring (RPM) to patients with congestive heart failure and those with a combination of diabetes and high blood pressure. RPM eligibility is based on individual medical diagnosis.

If you qualify for RPM through TCCH, our partner Accuhealth, will mail the appropriate equipment to your home at no cost to you. Once delivered, they will call with simple training on how to use the equipment.

Incentives, including gift cards, are available for patients who regularly participate in using their equipment. Ask the Accuhealth representative for incentive information when they call to set up your equipment.

Stay Connected to Your Provider

Remote Patient Monitoring

RPM transmits your information via Bluetooth technology while you stay comfortable in your own home. You do not need a cell phone to participate.

Licensed nursing professionals review
the data 24 hours, seven days a week
to monitor your health, make recommendations and follow up on high risk or critical values.

If a critical or abnormal value occurs, TCCH licensed nursing staff and medical providers will determine a course of action. This may include a visit to your medical provider, change in medication or instructions to go to the hospital.

Other TCCH Programs That Can Help You Succeed

Annual Wellness Visits

During this visit you will fill out a "Health Risk Assessment." Your provider will work with you to create a personalized health plan, based on questions about your medical history, current prescriptions and vital signs. TCCH does not charge a Medicare deductible for office visits with a medical provider.

Diabetes Program

A Certified Diabetes Care and Education Specialist listens to what you want and offers a variety of avenues to achieve optimal health.

On-site Pharmacy

Taking your medication as prescribed leads to better health. Affordable medicine makes it easier to stay healthy.

Vision Services

High blood pressure and diabetes can cause negative impacts on eye health. Board-certified optometrists diagnosed eye diseases such as macular degeneration, cataracts, diabetic retinopathy and glaucoma. TCCH offers very affordable eye exams and glasses.
